Radicular groove of maxillary premolar: Is a “danger zone"?

Objectives: To evaluate the presence of radicular groove and dentin thickness on palatal aspect buccal root maxillary first premolars using cone-beam computed tomography (CBCT). Materials methods: Images 312 belonging to 187 patients (between 18-69 years, 94 females 93 males) who were referred clinic retrospectively reviewed. Root canal treatment, periapical lesion, post-core excluded. One three-rooted also CBCT images viewed axial plane detect grooves. The concave area was recorded as a groove. Buccal thicknesses measured by two observers at level 3 mm below furcation. Statistical analyses performed. Results: 1.28(±0.25) 0.87(±0.13) mm, respectively. According Student’s t-test, statistically higher than (p=0.016). prevalence 82.05%. While without 0.93(±0.14) corresponding 0.82(±0.12) mm. One-way ANOVA showed in group 1 (18-35 years) lower (>65 (p=0.012 p=0.003). No statistical difference observed between sex left or right side (p>0.05). Conclusions: Palatal related can be considered “danger zone” for endodontic treatment. Considering high (82.05%) thin groove, more conservative post space preparation examination are recommended avoid perforation.
